Please read every section. Questions? Call or text 773-419-0823.
Draft template. This wording is a starting point, not legal advice. Have an Illinois attorney review it before clients sign.
1. Safety guidelines
- Tell your trainer about any medical condition, medication, surgery, pregnancy, or injury before every session.
- Stop right away and tell your trainer if you feel chest pain, dizziness, shortness of breath, nausea, or sharp pain.
- Wear proper athletic shoes and clothing, stay hydrated, and eat something light before training.
- Use equipment only as shown by your trainer. Never lift heavy weights without a spotter.
- Your trainer may change or stop any exercise at any time for your safety.
2. Injury reporting
- Report any injury, pain, or new symptom to your trainer immediately — during or after a session.
- If an injury happens, training on that area stops until you are cleared to continue.
- Your trainer may ask for written clearance from your doctor before you return to training.
- In an emergency, 911 will be called. You are responsible for any medical costs.
3. Medical clearance
- Getting Better Health & Wellness does not diagnose or treat medical or mental health conditions.
- You are encouraged to see your doctor before starting any exercise program, and you agree to do so if your trainer asks.
- You confirm the health information you share is true and complete, and you will report changes.
4. Assumption of risk
- Exercise carries risks, including muscle strains, sprains, fractures, heart problems, and in rare cases serious injury or death.
- You take part voluntarily and accept these risks, whether training indoors, outdoors, or in a small group.
5. Release of liability
- To the fullest extent allowed by law, you release Getting Better Health & Wellness, Javid L. Jenkins, and its trainers from claims for injury, illness, or loss connected with training, except those caused by gross negligence or willful misconduct.
- You agree this waiver applies to all current and future sessions until you withdraw it in writing.
